How do you authorise your contractor accountant to act on your behalf with HMRC?

When you sign up to a new contractor accountant, one of the first things you'll be asked to do is to authorise the accountant to act on your behalf with HMRC.
To do this, you will need to complete form 64-8 - 'Authorising your agent' which you can download here (PDF), or you can order the form online here.
Alternatively, your accountant can use HMRC's online authorisation service to avoid having to deal with the paper form. Either way, your new accountant will tell you how to complete the authorisation process, most likely when you first sign up to their service.
Despite authorising your accountant to deal with HMRC on your behalf, you are ultimately responsible for ensuring that your accounts are accurate and are filed within the statutory deadlines.
